Three-part Clinical Question:In patients with crush injury, does the addition of hyperbaric oxygen to standard therapy, compared to standard therapy alone, result in increased VEGF serum and mRNA levels?

Search Terms:Crush injury; vascular endothelial growth factor; wound healing

The Study:Non-blinded non-concealed randomised controlled trial with intention-to-treat.

The Study Patients:Patients between 15-64 years old admitted with crush injury. Patients with contraindications to receiving HBO were excluded.

Control group(N = 8; 8 analysed): Wound debridement and limb salvage surgery.

Experimental group(N = 7; 7 analysed): Wound debridement and limb salvage surgery followed by HBOT 100% O2 at 2.4 ATA for 60 minutes daily for 10 consecutive days.

 

The Evidence:

No comparisons made between groups after HBO course.

 

Comments:

  1. No outcomes assessed with which to compare HBO plus standard care to standard care alone.
  2. The sample size was very small with no sample size calculation described.
  3. Follow up period was limited to 0 days in the control group and 10 days in the hyperbaric group. No long-term conclusions can be drawn.
  4. The authors’ analysis looked at serum VEGF and VEGF mRNA. It is not clear why they did not compare groups at the 10 day point.
