Hyperbaric oxygen induced a greater percentage increase in doppler flowmetry in cheek and gingiva on heat provocation and may improve transcutaneous oxygenation when oxygen breathing.

Clinical Bottom Line: 1. The percentage change from baseline in transcutaneous oxygen on the administration of 100% oxygen (1ATA) was greater after HBOT than control. The absolute difference was not statistically significant between groups. |

Three-part Clinical Question:For adult patients with a history of irradiation on the face, does a course of hyperbaric oxygen therapy versus no treatment, improve vascular function and transcutaneous oximetry?

The Study:Non-blinded concealed randomised controlled trial with intention-to-treat.
The Study Patients: Patients with irradiation > 50Gy for the treatment of head and neck cancer.
Control group (N = 8; 8 analysed): No specific therapy aimed at improving perfusion.
Experimental group (N = 14; 14 analysed): 100% oxygen breathing at 2.4 ATA for 90 minutes daily, five days per week to a total of 28 treatments.

Comments:
1. Small study with low power to demonstrate between group differences. A number of within-group statistically significant increases from baseline in the HBO group.
2. Laser flowmetry measures perfusion of the tissue concerned.
3. All changes were in the direction of improvement with HBOT.
4. Authors also reported a small but significant improvement in cheek blood flow over time in the control group.
