Three-part Clinical Question:For healthy individuals older than 63, does a course of hyperbaric oxygen, compared to no treatment, result in any cognitive enhancement?
Search Terms: Ageing; dementia; cognition
The Study:Non-blinded randomised controlled trial without intention-to-treat.
The Study Patients: Healthy individuals aged >63 years.
Control group (N = 35 ; 32 analysed): No intervention or contact.
Experimental group (N = 35 ; 29 analysed): 100% oxygen breathing at 2 ATA for 90 minutes daily (Mon to Fri) to a total of 60 sessions.

Comments:
1. Study based on the association of ageing with both cognitive decline and falling regional cerebral blood flow. Authors assume cause and effect in operation.
2. 30 of 100 individuals contacted did not participate and nine of the remaining 70 were not included in results.
3. Short-term outcome at end of treatment period only. Long course of hyperbaric – 60 treatments. Not clear how this was chosen. It is not clear what min and max scores are possible from the outcome measures used (commercial tools).
4. No between-group comparisons are given, only the findings within each group. Some baseline differences between groups might be important (e.g. executive function 64.9 (control) versus 70 (HBO)).. Sample size based on five point improvement in cognitive function with HBOT versus 2 points in control. It is not clear the authors have tested for this differnce. They used a group by time interaction test rather thana simple comparison of means as suggested in their analysis plan.
